CELDr. Coles E. L'Hommedieu, President of Signature Medical Group, has been named a 2026 Titan 100 in St. Louis.

The Titan 100 recognizes St. Louis’s Top 100 CEOs and C-level executives, the region’s most accomplished leaders who have not only built extraordinary companies but have redefined what it means to lead. These Titans exemplify excellence through their vision, passion, and integrity, creating lasting impact across industries and communities.

Collectively, the 2026 St. Louis Titan 100 honorees and their companies generate more than $18 billion in annual revenue and employ over 38,000 individuals across the region and beyond, with more than half exceeding $25 million in annual revenue.

Dr. Coles L’Hommedieu joined the Signature Organization in 2004, fresh from training as an orthopedic surgeon with a specialization in hip and knee replacement. His clinical reputation quickly fueled the growth of his practice, but his ambitions extended beyond surgery.

Recognizing the importance of shaping the future of independent physician groups, he began to take on leadership roles within Signature, eventually serving on the Board of Directors before being named president in April 2019. Since then, he has guided the organization through a rapidly changing health care landscape – including the unprecedented challenges of the COVID-19 pandemic – while keeping its mission and independence at the forefront.

One key factor that sets Dr. L’Hommedieu apart from his peers is his over 20 years of consistently high surgical volume. While most orthopedic surgeons in the region average around 400 cases annually, Dr. L’Hommedieu performs approximately 900 procedures, almost all of them total joint replacements. Over his career, he has completed more than 10,000 total joint procedures; this figure far exceeds the national average and underscores his extraordinary experience, skill, and stamina.

Beyond his clinical practice, Dr. L’Hommedieu has contributed to the advancement of orthopedic surgery on a global scale. He has been consulted for implant design and process development by industry leaders such as Enovis, LIMA, United Orthopedics, DePuy, and Zimmer-Biomet. In 2021, he founded CL Medical Technologies, a company focused on creating and producing non-implantable orthopedic devices for both operating rooms and office-based procedures. By prioritizing speed to market, CL Medical Technologies shortens the time from concept to implementation, enabling widespread adoption of innovative tools.
