Signature Medical Group experience with bundled payments charts a path to more cost-effective care
St. LOUIS – John Tessier, MD; Gerald Rupp, PhD, and Jim Gera, MBA, have published an article on orthopedic value-based care and the impact of using defined care plans. The article published in the April 2016 issue of The Journal of Arthroplasty is entitled Physicians With Defined Clear Care Pathways Have Better Discharge Disposition and Lower Cost. The study uses data from Medicare patients in the Bundled Payments for Care Improvement initiative to compare medical costs and outcomes in lower joint surgery for physicians using well-defined care plans to those physicians without well-defined plans. Defined care plans include protocols to improve care before, during, and through 90 days after surgery. In the study, surgeries performed by physicians using defined care plans show reduced costs and improved patient outcomes.
The results of the study indicate the use of defined care plans saved $3,189 per hip replacement and $2,466 per knee replacement. Total lower joint replacements are projected to increase 673% by 2030. This study directly reports the effect of clinically proven care plans and shows how total joint protocols can produce overall cost savings while improving patient care and provide a means to meet the demand of future joint replacements. Effectively managed bundled payment programs led by physicians improve patient care and reduce healthcare cost.
