For close to a year, Barry Pointe has participated in Medicare's Comprehensive Primary Care Plus (CPC+) program. We are among the more than 2,800 primary care practices nationwide that entered the five-year program on Jan. 1, 2017. The Centers for Medicare and Medicaid Services says CPC+ is the largest ever initiative to improve primary healthcare in the U.S.
The goal of CPC+ is to provide doctors the freedom to care for their patients in a manner that produces positive health outcomes as well as lower health costs.
Barry Pointe also is proud to be a Level 3 Patient-Centered Medical Home and has earned re-credentialing for another three years. The Patient-Centered Medical Home is a model of care that puts patients first. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCHMs improve quality, the patient experience and staff satisfaction, while reducing health costs.
