Barry Pointe Family Care, a division of Signature Medical Group, received a designation of BDTC+ from the Blue Distinction Total Care Program. This Blue Cross and Blue Shield program recognizes primary care doctors, medical practices, and hospitals that participate in locally-tailored, patient-centered medical home (PCMH) models.
BDTC providers focus on:
- Providing person- and family-centered care.
- Supporting care that is continuous and comprehensive.
- Promoting care that is team based and collaborative.
- Orchestrating care that is coordinated and integrated.
- Delivering care that is accessible and of high value.
PCMH programs are intended to reduce cost and improve quality and efficiency while
recognizing value through enhanced reimbursement. BDTC providers receive payments known as performance-based incentives associated with both quality and cost outcomes.
Building on a data-driven and patient-centered approach, the BDTC program will expand to include a plus (“+”) designation. Beginning January 1, 2019, Blue Cross and Blue Shield will recognize BDTC providers who deliver high-quality, more affordable care to their patients. Only providers who meet the required program selection criteria for BDTC may be considered for the BDTC+ designation.
PCMH entities that meet or exceed the 70th percentile in both the Resource Utilization and Clinical Quality evaluation scores may be designated as BDTC+. This designation is visible to members and employers accessing the Blue Cross Blue Shield National Provider Finder.
Barry Pointe Family Care scored in the 91st percentile for Resource Utilization (cost) and in the 100th percentile for Clinical Quality, and thus has been awarded the BDTC+ distinction. +
